When Returning Home or to an Assisted Living

Regardless of where your patient is returning from or to, accommodations may need to be made to increase services and care to ensure a smooth transition back home and return mobility and agility. Several services are available and are covered by Medicare.  Do as much research as possible before your loved one has returned.  

 

For temporary support services consider reaching out to the Case Worker at Jenks Senior Center in Winchester or Minuteman Senior Services.

 

  • Jenks Senior Center's Case Worker Suzanne Norton provides referral information on temporary in home care services, free or low cost medical equipment rental, and caregiver support services.  The Jenks Center also offers SHINE (Serving the Health Insurance Needs of Everyone) counseling, which helps seniors navigate the health insurance system. snorton@winchester.us, Learn more.
  • Minuteman Senior Services is a non-profit agency that helps people of all abilities and backgrounds navigate aging. They provide a variety of services, including home care, family caregiver support, and rights advocacy, primarily serving residents in 16 towns north and west of Boston. Learn more.
  • To find similar services in other parts of the country, you can use the National Council on Aging's website to locate Area Agencies on Aging (AAAs) in your specific state or region. AAAs, like Minuteman Senior Services, are federally designated agencies that provide a range of services for older adults, including information and referral, home-delivered meals, and transportation.

Winchester Hospital offers traditional inpatient care, but also incorporates a Hospital at Home program to deliver hospital-level care in the comfort of the patient's home. This program allows some patients with acute illnesses or conditions to receive treatment and monitoring in their homes, rather than in a traditional hospital setting.  Other hospitals around the country are also beginning to utilize this approach. Ask your local hospital if they have a similar program.
